What determines when a line change is made?

The correct way is to shoot the puck into the other team's end of the rink softly, to give the forwards time to get off and a fresh forward line to get on the ice. The defense changes on the fly or at the next whistle, but not at the same time as the forwards do.
In a pro game, the lines are changing as often as every 45 seconds, if the play is flowing back and forth quickly, with only a few stoppages of play. On a power play situation, the coach wants his "best shooting line" to be out there, as much as possible during the penalty .
